It sounds like you already have created a PDF with form fields
using Acrobat or other desktop application. If so, then here is
what I do:
var('blank_pdf' = pdf_read(-file='/path/to/blank.pdf'));
var('completed_pdf' = '/path/to/completed/' + $something +
'.pdf'); // $something can be a unique identifier or whatever
$blank_pdf->setfieldvalue(-field='field1', -value=$value1);
...
$blank_pdf->setfieldvalue(-field='fieldN', -value=$valueN);
$blank_pdf->save(-file = $completed_pdf);
Then $completed_pdf can be served or emailed or whatever.

the pdf_read tags.
